What are the responsibilities and job description for the Resource Specialist/ADRC Counselor position at Three Rivers Regional Commission?
**This position will be located within the 10 county region based on your location your office could be located between Griffin and Franklin, GA.**
Primary Responsibility:
The ADRC Counselor provides information, assistance, and referrals for the elderly and disabled in the TRRC 10 county service area. In addition, this position is responsible for conducting screenings/assessments to determine a referral's potential eligibility for Medicaid Waiver Programs and HCBS services and is the first point of contact for individuals requesting assistance
The Resource Specialists maintains the state and nationally viewed elder/disabled resource database and identifies new community resources,
Summary of Duties:
ADRC Counselor:
· Utilizing a person-centered approach, process client assistance requests via telephone, TRAAA website, and walk-ins.
· Complete initial client screenings and level of care determinations for Medicaid Waiver Programs and HCBS services; complete monthly rescreens to assess client's health and functional status.
· Make referrals to case management, APS, DBHDD, GASHIP, Wellness Coordinator, and NH Care Transitions when appropriate.
· Assist with ADRC outreach activities.
Resource Specialist:
· Follow quarterly updating schedule assigned by Education and Resource Coordinator.
· Identify new resources that meet criteria for inclusion via print media, internet, and contact with the community.
· Maintain current knowledge of elder/disabled programs and resources.
· Assist staff with resource requests.
· Submit required reports reflecting database management to supervisor.
Resource Specialist/ADRC Counselor:
· Participate in conferences and trainings as requested by TRAAA/DAS.
· Process assistive technology requests and coordinate distribution to clients.
· Manage high stress situations.
· Perform required assignments with self-direction and minimal supervision.
· Complete other work-related duties as assigned by the ADRC Program Manager.
Qualifications and Requirements:
Bachelor's Degree in social work, sociology, psychology, human services, or a related field. Must obtain AIRS certification.
Other Requirements:
Pre-employment drug screen; criminal background check; proof of valid drivers’ license; conformity to Three Rivers Regional Commission standards of conducts for employees; professional business appearance and demeanor; appropriate language skills to communicate with a variety or business organizations and people; knowledge of program guidelines, human service providers, and community resources; understanding of human development and behavior as it relates to the elderly and disabled.
Knowledge Required by the Position:
Knowledge of Three Rivers AAA programs, policies, and procedures.
Knowledge of federal and state aging services guidelines.
Knowledge of relevant state and federal laws and agency rules and regulation as applied to aging services.
Knowledge of program guidelines, human service providers, and community resources.
Understanding of human development and behavior as it relates to the elderly and disabled.
Possess skills to effectively communicate with providers and clients to obtain updated information for database, provide information/referral assistance, and conduct client assessments.
Ability to research potential services/programs in our service area to be included in the database.
Skill in operating office equipment such as a computer and calculator.
Skill in public and interpersonal relations.
Skill in oral and written communication.
