This Position Assignment Addendum supplements the Three Rivers Public Health Department Community Health Worker I, II, and III job descriptions. All qualifications, essential functions, competencies, physical requirements, employment conditions, and expectations outlined in the applicable Community Health Worker job description remain in effect unless otherwise specified in this addendum. Applicants and employees should review the applicable Community Health Worker I, II, or III job description in conjunction with this addendum. The purpose of this addendum is to outline the unique responsibilities, work environment, partnerships, and expectations associated with the Methodist Fremont Health Community Health Worker assignment.

Position Assignment

This position is assigned to Methodist Fremont Health through a collaborative partnership with Three Rivers Public Health Department. The Community Health Worker (CHW) will be embedded within Methodist Fremont Health while remaining an employee of the Three Rivers Public Health Department.

Position Purpose

The Community Health Worker serves as a trusted liaison between patients, healthcare providers, the Three Rivers Public Health Department, and community organizations. This position focuses on improving access to healthcare, addressing social drivers of health, reducing barriers to care, improving care transitions, and connecting individuals and families with the resources they need to achieve better health outcomes.

Primary Responsibilities

In addition to the responsibilities outlined in the applicable Community Health Worker I, II, or III job description, this position will:

• Collaborate with physicians, nurses, social workers, care managers, case managers, discharge planners, and other Methodist Fremont Health staff.

• Receive referrals from hospital staff and assist patients in navigating healthcare and community resources.

• Conduct screenings and assessments related to social drivers of health.

• Connect patients with transportation, housing, food resources, financial assistance, behavioral health services, insurance enrollment, and other community resources.

• Provide patient navigation, education, advocacy, coaching, and follow-up services.

• Support safe transitions of care following hospitalization or emergency department visits.

• Participate in interdisciplinary care coordination meetings.

• Build strong partnerships between Methodist Fremont Health, Three Rivers Public Health Department, and community organizations.

• Document services according to Three Rivers Public Health Department and Methodist Fremont Health requirements.

• Participate in required Three Rivers Public Health Department meetings, Community Health Worker meetings, trainings, and professional development activities.

Work Environment

The Community Health Worker will primarily work on-site at Methodist Fremont Health while also participating in meetings, training sessions, and regional Community Health Worker initiatives at the Three Rivers Public Health Department.

Assignment Flexibility

Three Rivers Public Health Department reserves the right to modify work assignments, partner locations, or primary work sites based on organizational needs, funding requirements, community priorities, and program objectives while maintaining the employee's classification as a Community Health Worker I, II, or III.
