Job Overview
The Intake Coordinator is an on call position and plays a crucial role in the healthcare setting by managing the intake process for patients. This position involves coordinating patient information, ensuring compliance with medical standards, and facilitating communication between patients and healthcare providers. The ideal candidate will have a strong background in hospital operations, patient care, and relevant clinical experience.

Duties

• On Call Weekend availability From Friday 8:30PM- Monday 8:00AM
• Manage the intake process for new patients, including gathering medical history and relevant documentation.
• Conduct patient assessments to determine care needs and eligibility for services.
• Collaborate with healthcare professionals to ensure accurate and timely patient information is processed.
• Maintain compliance with HIPAA regulations and NCQA standards while handling sensitive patient data.
• Coordinate with various departments to facilitate seamless patient transitions through different levels of care.
• Provide education to patients regarding their treatment options and assist them in navigating the healthcare system.
• Support case management efforts by communicating effectively with patients, families, and healthcare teams.
• Assist in scheduling appointments and follow-up visits as necessary.

Qualifications

• Experience in a hospital setting or related field is preferred.
• Familiarity with ICD-9/ICD-10 coding and medical terminology is essential.
• Clinical experience in areas such as psychiatric care, Long term care, or geriatrics, is highly desirable.
• Strong communication skills with the ability to work effectively with diverse populations, including those with developmental disabilities or senior care needs.
• Ability to perform vital signs assessments and basic life support as required.
• Experience working in various healthcare environments such as hospitals (Level I/II trauma centers), nursing homes, or outpatient clinics is an asset.
• A commitment to providing compassionate care while adhering to ethical standards of practice.
• This role requires a proactive individual who can thrive in a fast-paced environment while maintaining attention to detail. The Intake Coordinator will be instrumental in ensuring that patients receive the highest quality of care from their initial contact through their ongoing treatment journey.
• Must possess a RN/MSW License.
