Full Time & Part Time: $32-$37.25/hour
(based on experience & shift differentials)
Per Diem: $27-$31/hour
(based on experience and shift differentials)
Pick-Up Pay: $5/hour weekdays | $7/hour weekends

Full Time | Day Shifts
Part Time | Day & Evening Shifts
Per Diem | Minimum two shifts per month

What You'll Do as an LPN:
- Safely provide medications and treatments as prescribed by physicians, following accepted standards of practice and organizational policies.
- Maintain accurate, timely written and electronic records in compliance with legal, regulatory, and company requirements.
- Monitor and document residents' conditions, report changes to the RN and relevant team members, and collaborate in developing and implementing individualized care plans.
- Assist with admissions and readmissions, gather clinical data, support resident care programs by ensuring adherence to plans of care, and oversee CNA performance in these areas.
LPN Requirements:
- Active Wisconsin Licensed Practical Nurse (LPN) license
- Current CPR certification
- Long-term care experience preferred
- Basic computer skills
- Ability to push/pull up to 60 lbs. and occasionally lift up to 100 lbs.
