Assisted Living LPN
Part Time | Day or Night Shift
** PAY RATE BETWEEN $31-36.75/HOUR **
(based on experience and shift differentials)

Assisted Living LPN Requirements:
- Licensed Practical Nurse (LPN) license in Wisconsin.
- Current and valid CBRF certification.
- Experience in assisted living care is preferred.
- Basic computer knowledge.
- Knowledge of DHS 83, state, and federal regulations related to long-term care preferred.
What you'll do as an Assisted Living LPN:
- Safely provide medications and treatments per physician orders and organizational policies.
- Maintain accurate records, observe resident conditions, and report changes to the care team, including collaboration on Individualized Service Plans (ISPs).
- Assist with admissions, provide direct care, and support care programs such as bowel/bladder, skin integrity, and pain management, ensuring care plans are followed.
- Work closely with the interdisciplinary team and oversee CNA responsibilities in care program areas to ensure quality resident care.
