Memory Care Nurse: RN or LPN
Full Time and Part Time | Day and Evening Shifts
** PAY RATE BETWEEN $31-41/HOUR **
(based on experience and shift differentials)

Nurse Requirements:
- Current and valid license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Wisconsin.
- Current and valid CPR certification.
- Previous experience in long-term care is preferred.
- Basic proficiency in computer use and electronic health record systems.
- Physically capable of frequently pushing or pulling objects weighing up to 60 pounds, and occasionally lifting or moving objects weighing 100 pounds or more.
What you'll do as a Nurse:
- Administer Medications and Treatments: Safely deliver prescribed medications and treatments in accordance with physician orders, regulatory standards, and organizational policies.
- Monitor and Document Resident Conditions: Observe residents, accurately document findings, and communicate changes in condition to the appropriate healthcare team members.
- Support Care Planning and Admission Processes: Collaborate with RNs and interdisciplinary teams in developing, implementing, and updating individualized care plans, including during admissions and readmissions.
- Participate in and Oversee Resident Care Programs: Actively support resident care initiatives (e.g., skin integrity, pain management) and ensure CNAs fulfill their responsibilities within these programs.
