Looking for RN Case Manager in Glenwood Springs, CO,
13 weeks contract,
Shift: 3-12's or 4-10's YOU CHOOSE! This is a full-time day position with weekend rotation, no call.

GENERAL OBJECTIVES:
- Assists Valley View inpatients and outpatients and their families in the overall Management of their plan of care across the continuum.
QUALIFICATIONS:
- ·Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program
- · State licensure as a Registered Nurse (RN).
- · Experience in Case management is required.
