Minimum 3 years recent acute care RN experience (Care Coordination experience required – CommonSpirit/Dignity/CHI background mandatory) / CERTIFICATIONS – BLS (Required) / LICENSE – Active CA RN license required at submission / Schedule: Days, 8s, 40 hrs/week, 13 weeks / Unit: Case Management / Care Coordination – Inpatient / EMR: Cerner / Responsibilities: Discharge planning, initial assessments, coordination of post-acute care (SNF, home health, DME, prior authorizations), managing diverse patient populations (including uninsured/homeless) / Skills: Strong communication, multitasking, independent work in fast-paced environment / Caseload: 15–28 patients/day / Dress Code: Business casual with lab coat / PAY IS NEGOTIABLE!
