Job Description
Home Health Care Solutions is seeking a Full-Time Registered Nurse to provide skilled nursing care to patients within the LaPorte area. Pay is based on completing 30 units per week, which is estimated to be approximately 40 hours of work per week.

Key Responsibilities
- Provide skilled nursing care and case management in a home health setting.
- Collaborate with an interdisciplinary team to develop and implement individualized care plans.
- Perform patient and family education to promote independence and optimal clinical outcomes.
- Maintain accurate and timely clinical documentation.
Requirements
- License: Current RN license in the State of Indiana.
- Skills: Strong clinical assessment, communication, and organizational skills.
- Experience: Home health experience is preferred but not required.
- Reliability: Valid driver’s license and dependable transportation for travel within the LaPorte territory.
