  • Collaborate with the interdisciplinary team to maximize patient outcomes, functional independence, and successful transitions of care.
  • Maintain timely, accurate clinical documentation consistent with professional standards and regulatory requirements.

Therapy Team Leadership & Coordination

  • Serve as the primary day-to-day liaison between the therapy department, facility leadership, nursing staff, and interdisciplinary care team.
  • Coordinate daily therapy operations and assist with patient and therapist scheduling to support timely, efficient delivery of services.
  • Work closely with therapy staff and MDS personnel to coordinate treatment schedules, patient needs, and care planning.
  • Represent the therapy department at facility meetings, care conferences, and interdisciplinary discussions.
  • Promote positive communication, teamwork, accountability, and a patient-centered culture.
  • Identify operational challenges and collaborate with Theracare's regional leadership team to implement effective solutions.
  • Help maintain strong relationships with facility staff, patients, families, and referral sources.
  • Support the continued success and growth of the facility's established outpatient rehabilitation program.

  • Experience with outpatient rehabilitation services is a plus.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to coordinate multiple priorities in a busy rehabilitation environment.
  • Demonstrated initiative, sound judgment, and the ability to foster positive working relationships.
