Job Summary:
TheraCare Home Health is seeking a detail-oriented and proactive Licensed Vocational Nurse (LVN) Clinical Coordinator to support clinical operations in an in-office setting. In this role, you will ensure critical clinical tasks are tracked, communicated, and resolved efficiently to support high-quality patient care. You will work closely with the Director of Clinical Services (DCS) and clinical team to manage workflows, monitor documentation, and facilitate timely follow-up on labs, physician orders, and patient-related communications. This role is essential in maintaining compliance, improving efficiency, and ensuring nothing falls through the cracks in the home health process.
Qualifications:Qualifications:
- Current, unrestricted Licensed Vocational Nurse (LVN) license
- Minimum of 2 years of home health experience required
- Strong knowledge of home health documentation, physician orders, and regulatory requirements
- Experience with Homecare Homebase (HCHB) required
- Familiarity with Forcura, Mosaic, or similar document management systems
- Ability to manage multiple tasks, follow-ups, and priorities in a fast-paced environment
- Strong attention to detail and organizational skills
- Excellent communication skills with clinicians, physicians, and leadership teams
- Ability to work independently while supporting a collaborative clinical environment
Essential Functions:
- Track and follow up on lab results, diagnostic reports, and clinical findings to ensure timely resolution
- Identify abnormal or overdue labs and escalate concerns to the Director of Clinical Services (DCS) or appropriate clinician
- Ensure lab results and updates are communicated to physicians and documented accurately
- Monitor patient charts to confirm all follow-up actions are completed and compliant
- Answer and triage clinical phone calls from field clinicians, patients, and caregivers
- Route urgent or complex clinical issues to RN leadership or DCS per protocol
- Document all clinical communications and ensure issues are resolved efficiently
- Monitor and manage HCHB task queues and Forcura/Mosaic workflows daily
- Track outstanding items including physician orders, Face-to-Face (F2F) documentation, medical records requests, and unsigned documents
- Proactively follow up with clinicians, intake, QA, and physicians to prevent delays in care or documentation
- Review Face-to-Face documentation for completeness and accuracy
- Assist with preparation and tracking of physician orders from initiation through signature and upload
- Coordinate corrections, clarifications, and follow-up actions related to clinical documentation
- Support the Director of Clinical Services with clinical tracking, workflow oversight, and process improvement
- Identify workflow gaps, delays, and trends, and communicate findings to leadership
- Assist with chart preparation for audits, surveys, and quality assurance reviews
- Promote efficient workflows, compliance, and high standards of patient care
