Responsibilities

• Perform comprehensive patient evaluations and develop individualized treatment plans
• Deliver hands-on, evidence-based physical therapy interventions
• Monitor patient progress and modify treatment plans as appropriate
• Maintain timely and accurate documentation
• Collaborate with teammates and healthcare providers to optimize patient outcomes
• Develop discharge plans and support long-term patient success
• Supervise rehabilitation technicians as appropriate
• Participate in clinic education, mentoring, and community outreach initiatives
• Contribute to a positive, patient-centered clinic culture

Qualifications

• Graduate of an APTA or CAPTE-accredited Physical Therapy program
• Licensed Physical Therapist (or eligible for licensure) in Illinois
• Current Basic Life Support (BLS) certification
• New graduates are encouraged to apply and will receive mentorship and support
• Experienced clinicians are welcome and valued
