You'll provide high-quality, patient-centric primary care services by conducting rounds in skilled nursing facilities, diagnosing acute and chronic conditions, and documenting patient information in Theoria's proprietary EHR system, ChartEasy.
Required Skills and Qualifications
A successful candidate will possess strong clinical skills, effective communication abilities, and proficiency in electronic medical tools, including EHR systems and state/county medical systems. Participation in Theoria's quality initiatives, such as Chronic Care Management and RPM, is also crucial.
