We are looking for a Registered Nurse (RN) Clinical Manager to steer our clinical team and ensure our home health services remain top-tier. The Role: You’ll be the heartbeat of our clinical operations, balancing mentorship with administrative oversight. Your mission: oversee case management, ensure OASIS accuracy, and lead a multidisciplinary team to deliver high-quality care in the comfort of patients' homes.What You’ll Do
- Lead & Mentor: Supervise and support field staff (RNs, LPNs, Therapists) to ensure clinical excellence.
- Quality Oversight: Review patient charts and care plans for compliance with Medicare and state regulations.
- Case Management: Coordinate admissions, discharges, and transitions of care.
- Problem Solve: Act as the clinical point of contact for physicians, families, and staff.
What You Bring
- License: Current, unencumbered RN license in this state.
- Experience: At least 1 years of home health experience and 6 months in a leadership/supervisory role.
- Tech Savvy: Proficiency with EMR systems and a deep understanding of OASIS.
- Heart: A passion for community-based health.
