The Young House Adult Family Home is seeking a dedicated and organized Resident Care Manager to join our team. The ideal candidate will play a vital role in overseeing the care and well-being of our residents, ensuring smooth daily operations, and acting as a liaison between residents, families, caregivers, and healthcare providers. Key Responsibilities
Resident Care Coordination
- Manage and oversee daily care for residents, including health monitoring and care updates.
- Coordinate with families to address resident care needs, follow-ups, and changes in care plans.
- Schedule resident appointments; assist with wheelchair transportation and mobility.
- Set up transportation for residents as needed in coordination with families.
- Support admission and discharge processes for residents, ensuring all documentation is accurate and timely.
Communication and Clerical Duties
- Maintain clear and professional communication with families, caregivers, and healthcare providers.
- Use platforms like Google Docs and Synkwise for documentation, record-keeping, and file management.
- Call doctors to schedule appointments, follow up on resident care orders, and manage pharmacy coordination for medication changes.
- Audit medical records and manage medication administration, ensuring compliance with policies and regulations.
Caregiver Support and Training
- Manage caregiver scheduling and provide updates on resident care needs.
- Report to the AFH provider on nurse delegation tasks and support needs.
- Assist with onboarding and training caregivers as required.
Administrative Oversight
- Ensure accurate documentation of all resident care activities, including Synkwise and other systems.
- Maintain up-to-date resident records, including medical records, medication changes, and care plans.
- Provide clerical support, including organizing documents, uploading files, and handling communications.
Qualifications
- Proven experience in caregiving, social services, healthcare, or a similar managerial role in an adult family home or care facility.
- Excellent communication and organizational skills.
- Proficiency in Google Docs and Synkwise or similar documentation platforms.
- Strong clerical skills, including record management, scheduling, and documentation.
- Ability to effectively coordinate with doctors, pharmacies, families, and caregivers.
- Compassionate and patient-centered approach to resident care.
Position and Duties
The Employee is hired as a Resident Care Manager (RCM) and agrees to perform duties including but not limited to:
- Resident care coordination, documentation, and compliance.
- Supervision and support of caregivers and staff.
- Weekend check-in duties, staff support, activity planning, and administrative tasks.
- Completion of all assigned tasks during scheduled shifts.
Work Schedule
- Regular Schedule: Monday through Friday, 8:00 AM – 4:00 PM, including a 30-minute paid lunch break.
- Weekend Rotation: Mandatory work rotation on the 1st and 3rd weekends of each month, and the 5th weekend if applicable.
- Weekend hours: 4 hour minimum each Saturday and Sunday for staff check-in, support, administration and planned activities.
- Employee receives an additional $1.00/hour for weekend shifts.
- Weekends Off: Employee is guaranteed the 2nd and 4th weekends off. Time off for working days request required in advance.
- Shift Adjustment: If a shift needs to be picked up due to open coverage, the Employee is required to adjust their weekly schedule accordingly.
Compensation
- Hourly Rate: $25.00 - $27.00 (BOE)
- Weekend Differential: $1.00 additional per hour for weekend hours worked.
- Overtime: Paid at 1.5 times the regular hourly rate for all hours worked over 40 in a workweek. Management approval required.
- Remote Work: If work is completed outside the facility (e.g., phone calls to staff, family, or medical professionals), Employee must record the time worked and submit with payroll records at the end of each pay period.
Attendance and Shift Requirements
- Employee is expected to clock in and clock out at the facility at the start and end of shifts.
- Shift pick-up on scheduled days is mandatory if coverage is needed; open shifts, call outs.
- Remote work must be clearly documented and recorded at the end of each pay period.
- Flexible work weeks allowed with communication for appointments and schedule balance.
- Employee is allowed to have another employer/occupation as long as it does not interfere with the RCM responsibilities & requirements.

- Maintain 100% compliance with resident care plans and staff training schedules.
- Ensure timely medication audits and accurate Synkwise documentation.
- Successfully manage staffing binders, onboarding processes, and credentialing deadlines.
- Achieve zero regulatory citations related to RCM responsibilities during the contract period.
- Submit monthly management reports and task completion summaries on time.
Preferred Skills and Abilities
- Experience with Case management, social resources, care and services delegation and medication management in a care home setting.
- Ability to manage multiple priorities and remain flexible to meet changing resident needs.
- Capability to assist residents with mobility and wheelchair use.
- Strong leadership and team management skills.
