Position Summary
Hours 9am - 730pm
Thursday - Sunday
The Caregiver / Activities Assistant plays a key role within the Lakewood Adult Family Home. This position is responsible for providing high-quality direct resident care, overseeing daily caregiving operations, ensuring medication and care plan compliance, supporting staff and managing the home’s daily and monthly resident activities program. This role requires strong organization, compassion, accountability, and the ability to lead by example.
Key Responsibilities Resident Care & Supervision
- Provide hands-on, direct care to residents in accordance with individual Negotiated Care Plans (NCPs)
- Support daily care delivery to ensure safe, dignified, and person-centered care
- Monitor resident condition changes and promptly report concerns to the Provider or Nurse Delegator
- Support ADLs including bathing, dressing, toileting, transfers, feeding, and mobility assistance
- Ensure infection control, skin care, repositioning, and fall prevention protocols are followed
- Serve as a consistent, calm presence for residents with dementia or behavioral support needs
Medication Management & Compliance
- Support medication administration following Nurse Delegation and the 7 Rights of Medication Administration
- Conduct routine medication audits (MARs, PRNs, counts, storage, labeling)
- Ensure medications are properly stored, locked, and documented
- Identify discrepancies or concerns and escalate immediately to leadership
- Assist with receiving medications from pharmacy and verifying accuracy
- Support caregiver adherence to delegation tasks and documentation standards
Care Plan & Resident Profile Compliance
- Ensure resident files, care plans, and profiles remain accurate and up to date
- Assist with care plan reviews, updates, and implementation
- Monitor daily documentation for completeness and compliance
- Support audits and inspections by maintaining organized, survey-ready records
- Collaborate with nursing, home health, hospice, and therapy providers as needed
Staff Responsibilities
- Assist with onboarding and orientation of new staff
- Reinforce proper caregiving techniques, safety practices, and house policies
- Complete ongoing education and competency compliance
- Model professionalism, teamwork, and respectful communication at all times
- Assist leadership with identifying training needs or performance concerns
On-Call & Shift Coverage
- Participate in on-call rotation as scheduled
- Pick up overtime or coverage shifts as needed to ensure resident safety
- Assist with smooth shift transitions and communication between staff
- Respond calmly and effectively to urgent situations or staffing gaps
Activities Assistant & Meaningful Day Program
- Plan, coordinate, and execute daily resident activities in compliance with the Meaningful Day program
- Develop and implement monthly activity themes
- Coordinate birthday celebrations, holiday events, and seasonal activities
- Organize and support safe outings when appropriate
- Maintain activity calendars, boards, and documentation
- Adapt activities to resident preferences, cognitive level, and physical abilities
- Encourage participation while respecting resident choice and dignity
Qualifications & Requirements
- Active NAR, HCA or CNA license in Washington State (required)
- Current Nurse Delegation (or ability to obtain)
- Dementia Specialty Training (required or willing to obtain)
- CPR/First Aid certification
- Minimum 2 years caregiving experience (AFH experience strongly preferred)
- Caregiver experience preferred
- Strong organization, documentation, and communication skills
- Ability to lift, transfer, and assist residents safely
- Reliable, flexible, and able to work independently and as part of a team

Job Type: Full-time
Pay: $18.00 - $20.00 per hour
