This full-time Social Worker role is an on-site position located in Paterson, NJ. The Social Worker will provide case management support and resources for individuals and families, including conducting assessments, creating care plans, offering counseling, and connecting clients to essential services. Additional responsibilities include maintaining accurate records, collaborating with community organizations, and conforming to confidentiality practices. The role requires working directly with diverse populations, including youth and homeless individuals, to address their specific needs and improve their quality of life.
- Strong case management and counseling skills, with experience conducting assessments and developing care plans
