Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation to develop individualized care plans.
- Administer medications, injections, IV infusions, and other treatments following strict aseptic techniques and safety protocols.
- Provide wound care, suturing assistance, ostomy management, and tube feeding to support recovery and comfort.
- Educate patients and families on disease management, medication usage, and health coaching to empower self-care at home.
- Monitor patient progress through regular documentation in electronic health record (EHR) systems such as Epic or Cerner; communicate updates with the broader healthcare team.
- Manage complex cases involving acute pain, infectious disease care, hospice & palliative medicine needs, or behavioral health issues with sensitivity and professionalism.
- Coordinate discharge planning and collaborate with case management to ensure seamless transitions across care settings like nursing homes or assisted living facilities.
Requirements
- Valid RN license in good standing with current state certification.
- Proven experience in home health nursing or related fields such as medical-surgical, ICU, emergency medicine, or long-term care.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-10/ICD-9), and diagnostic evaluation techniques.
- Proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks for accurate medical documentation.
- Ability to perform invasive procedures such as IV insertion, phlebotomy, catheterization, and specimen collection with precision.
- Excellent patient assessment skills combined with critical thinking for triage and urgent care situations.
- Familiarity with infection control practices and HIPAA regulations to ensure patient privacy and safety.
- Experience working with diverse populations including pediatrics, geriatrics, individuals with disabilities or developmental delays.
- Certifications in basic life support (BLS), advanced cardiovascular life support (ACLS), or specialized training such as ostomy care or dialysis is preferred but not required.
