The Willows at Ramapo Rehabilitation and Nursing Center offers the finest in comfort, care, and healing seven days a week in a unique spa-like setting. The facility is currently looking for a qualified, hard working and customer-service driven Registered Dietitian (RD) with long-term care experience to join our team.

Registered Dietitian Responsibilities Include but Are Not Limited To:

• Monthly documentation for weight changes, pressure ulcers, tube feeders and dialysis residents

• Monthly communication with renal RD's

• Complete nutrition assessments for long and short term residents and meal rounds

• Create, update and maintain care plans as well as attend care plan meetings

• Complete MDS books in a timely manner

• Obtain monthly weights and re-weights from all units

• Obtain weekly weights every Monday from short term units

• Prepare monthly weight report for DON

• Prepare monthly and quarterly QAPI reports

• Communicate any weight changes or declines in PO intake with MD & team

• Communicate with families and update food preferences in Meal Tracker

• Trouble shoot complaints received from families and residents

• Order GT feeding from MBS and provide all information for new residents, communicate any changes so we receive accurate GT feeding. Monitor inventory.

• Obtain and update food preferences with residents and families

• Review PRI's on new admits for diets and input into Meal Tracker

Registered Dietitian Qualifications/Requirements:

• Must hold current NYS CDN (Certified Dietician-Nutritionist) license

• Must hold current RD

• Experience in long-term care preferred

• Ability to multi-task and manage work autonomously

• Must be 18 years of age
