What You’ll Do
- Be the first trusted connection for patients and families entering hospice care
- Lead the admissions process with confidence, compassion, and professionalism
- Perform in-depth assessments and create individualized plans of care
- Educate and empower families so they feel supported, informed, and at ease
- Partner with physicians and interdisciplinary teams to ensure seamless care
- Provide real emotional support—not just clinical care
- Coordinate services and guide patients through every step of their journey
- Mentor and supervise support staff to maintain exceptional care standards
- Keep accurate documentation while managing your day with independence

- Current Registered Nurse (RN) license in the State of Texas
- At least 1 year of nursing experience (hospice or home health a plus)
- Excellent communication and interpersonal skills
- Strong organizational and critical thinking abilities
- Ability to work independently while thriving in a team environment
- Willingness to travel and take after-hours calls
- Ability to lift up to 55 pounds and perform physical job duties
- Commitment to compassionate, patient-centered care
