Key Responsibilities
- Assess patients' physical, emotional, and social needs to optimize care planning
- Coordinate communication between patients and providers to support continuity of care
- Guide patients through the healthcare system, ensuring timely access to education, treatment, and resources
- Track and document patient data in an accurate and organized manner
- Deliver direct patient care and perform clinical procedures within scope of training
- Provide education and support to patients and their families throughout the care continuum
- Conduct telephone follow-ups and patient communication within the RN scope of practice
- Collaborate with multidisciplinary team members to ensure a comprehensive approach to care
- Perform other duties as assigned
Education & Experience
- Required:
- Graduate of an accredited nursing program
- At least one (1) year of nursing experience in a medical, surgical, or clinical setting
- Preferred:
- Bachelor’s degree in nursing
- Experience in outpatient nursing and scheduling
- Current RN license in the state of employment (or compact license)
- BLS certification through the American Heart Association required for all new hires and transfers
- Knowledge of health system resources and patient navigation practices
- Strong written and verbal communication skills
- Conflict resolution and problem-solving ability
- Proficient in computer systems and documentation
- Compassionate and professional demeanor in all patient interactions
