The Wesley Community is seeking a compassionate and experienced Social Worker to join our team on a full-time basis. As a Social Worker, you will be responsible for providing social work services to residents in our long-term care units. About the Wesley Community:

Responsibilities:

• Provide social work services to residents in our long-term care units, including assessing social and emotional needs, developing and implementing care plans, and providing counseling and support.

• Maintain accurate and timely documentation of interactions with residents and families.

• Collaborate with other healthcare professionals to ensure coordinated care and services.

• Provide education and support to residents and families on topics such as long-term care options, insurance, and benefits.

• Develop and maintain relationships with community agencies and organizations to ensure access to resources and services for residents.

Requirements:

• Bachelor's degree in social work or human services field.

• Geriatric experience preferred.

• Strong communication and interpersonal skills.

• Ability to work independently and as part of a team.

• Must be able to work day shifts.
