Director of Community Outreach

The W Group
Brooklyn, NY Full Time
POSTED ON 8/1/2026
AVAILABLE BEFORE 8/30/2026
About The Opportunity

We're opening a brand new private pay assisted living community and looking for a Director of Community Outreach to help build it from the ground up. This is a founding team role with direct ownership over outreach and admissions strategy from day one, and a clear path to a broader regional role as our portfolio grows.

Position Summary

The Director of Community Outreach is a key ambassador for our company, responsible for identifying prospective residents, conducting screenings, and guiding families through the admissions process. The role includes leading community tours, marketing the community, and building referral relationships from scratch in a new market. Success means converting inquiries into move-ins and rapidly establishing census during our opening phase.

This is a strong fit for someone with proven sales or admissions instincts, a genuine passion for working with seniors, and the drive to launch something new.

Responsibilities

  • Build and maintain relationships with referral sources, including hospitals, skilled nursing facilities, senior organizations, community centers, and discharge planners
  • Lead outreach calls, community visits, networking events, and promotional activities to build market presence
  • Coordinate and conduct tours, interviews, and screenings for prospective residents
  • Manage the admissions process end to end, ensuring required medical, mental health, and financial documentation is complete and compliant with NYSDOH guidelines
  • Confirm transportation, communication, and documentation needs are resolved prior to admission
  • Serve as the liaison between families, referral sources, and community teams, resolving concerns quickly and professionally
  • Partner with corporate and community teams to hit occupancy goals, monthly tour quotas, and outreach targets, especially during pre-opening and stabilization
  • Provide daily updates on lead pipeline and conversion progress
  • Maintain working knowledge of NYSDOH regulations related to assisted living admissions
  • Keep CRM records current for all leads and tour activity
  • Uphold HIPAA standards and maintain strict confidentiality at all times

Qualifications

Education:

  • High School Diploma or equivalent required; Bachelor's degree in marketing, business, healthcare administration, or related field preferred
  • Fluent English required (read, write, speak)

Experience:

  • Healthcare sales, outreach, or admissions experience preferred; experience marketing to private pay clientele strongly preferred
  • Experience launching or building census for a new community is a plus

Other Requirements:

  • Valid driver's license and reliable transportation required for community visits, referral meetings, and offsite events
  • Availability for occasional evenings and weekends to accommodate tours and move-ins

Physical Requirements

  • Ability to move throughout the workday, including sitting, standing, walking, and local travel between sites
  • Occasional light lifting (marketing materials, event setup)

Compensation

$125,000 to $150,000 base, commensurate with experience, plus a performance-based bonus tied to occupancy and move-in goals.

Salary : $125,000 - $150,000
