- Educate patients and families about care procedures and health management.
- Collaborate with a multidisciplinary team providing comprehensive patient support.
- Active and current RN license.
- Preferred one year of clinical experience; prior home health experience is welcomed but not required, as training is provided.
- Ability to travel locally to patient cases.
