Seeking a Transplant Coordinator to join the transplant team, collaborating with physicians, surgeons, and advanced practice providers for inpatient and outpatient pre- and post-transplant patient care.
- Coordinate multidisciplinary evaluations, update candidate assessments based on current standards, and participate in patient selection decisions.
- Manage post-transplant patient care, handle urgent evaluations, and provide recommendations for policy and protocol improvements through literature review.
- Serve on patient selection committees and make candidacy recommendations.
- Must become certified as a Certified Clinical Transplant Coordinator within two years.
- Minimum of 2 years applicable nursing experience.
- Registered Nurse licensure with a Bachelor's Degree in Nursing.
- Strong collaborative and communication abilities.
- Knowledge of transplant standards of care and policy development.
- Ability to manage complex patient needs and participate in multidisciplinary teams.
