Physical Therapist (PT) | Outpatient | Human Performance Center

Role: Responsible for evaluating, developing, and delivering individualized physical therapy services to patients, ensuring treatment aligns with clinical goals and patient needs.

Key Responsibilities

• Assess patients using various data sources and develop personalized treatment plans.
• Implement therapy strategies to promote health and achieve treatment objectives.
• Provide direct therapy in diverse settings, coordinating with multidisciplinary teams.
• Document evaluations, treatment plans, progress notes, and communicate effectively with patients, families, and healthcare team members.
• Build rapport with patients and participate in ongoing professional development.
  
  

Qualifications & Skills

• Active physical therapy license in the practice state.
• Ability to perform job functions, including effective communication and visual acuity.
• Preferred 1-3 years of experience as a licensed physical therapist.
