Role Description
This is a full-time, on-site role based in Vancouver, WA for an Ambulatory Clinical Pharmacist. The Ambulatory Clinical Pharmacist will be responsible for delivering evidence-based pharmaceutical care to patients in an outpatient setting. Responsibilities include reviewing medications, providing clinical consultations to optimize drug therapy, supporting chronic disease management, compounding medications as necessary, and ensuring patient safety through proper medication use. Coordination with other healthcare professionals to develop personalized treatment plans will also be a key aspect of the role.
Qualifications
- Expertise in Pharmacy Practice and Pharmaceutical Care to ensure optimal medication management
- In-depth knowledge of Pharmacy and Medicine to offer accurate clinical evaluations and consultations
- Experience in Compounding and preparing customized medications for specific patient needs
- Strong interpersonal and communication skills for collaboration with healthcare teams and patient interaction
- Doctor of Pharmacy (Pharm.D.) degree from an accredited institution
- Valid pharmacist licensure in the state of Washington
- Previous experience in an ambulatory or outpatient care setting preferred
