Minimum Qualifications:
- Completion of a physician assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA)
- No experience required
Required:
- Certification by the National Commission on Certification of Physician Assistants (NCCPA)
- Licensed Physician Assistant by the Texas Physician Assistant Board/Texas Medical Board (TPAB/TMB)
NURSE PRACTITIONER
EDUCATION & EXPERIENCE
Minimum Qualifications:
- Completion of a graduate level (or higher) nurse practitioner program accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE)
- No experience required
Required:
- Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP)
- Licensed Registered Nurse (RN) by the Texas Board of Nursing (TBON)
- Licensed Advanced Practice Registered Nurse (APRN) in a nurse practitioner clinical area of specialization by the Texas Board of Nursing (TBON)
Job Duties:
- Provide comprehensive primary service medical management for patients admitted to the hospice unit.
- Conduct patient assessments, develop and update care plans, and collaborate closely with interdisciplinary team members.
- Support seamless transitions of care between acute care services, ICU, ED, and the hospice unit.
- Participate in family meetings, goals of care discussions, and coordination of comfort focused treatment plans.
- Ensure high quality documentation, regulatory compliance, and adherence to hospice care standards.
- Contribute to unit development, workflow optimization, and quality improvement initiatives.
- Serve as a clinical resource and support education for staff, trainees, and collaborating
