General Purpose of Job:

The Rehabilitation PCT provides assistance and basic care, as well as to reinforce and maintain the patient’s therapeutic regime around the clock.

Essential Duties:

This job description is intended to cover the minimum essential duties assigned on a regular basis. Associates may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.

• Performs specific patient procedures and treatments according to hospital and unit policy.
• Assists patients with dressing, use of equipment, donning and doffing support clothing, orthotic / prosthetic devices, and or protective devices.
• Assist patient with bathing, hygiene and other personal care activities.
• Assists with patient meals and provides reinforcement of feeding training as prescribed by SLP, OT, RN.
• Assists in managing intake and output schedules / programs.
• Assist with carrying out bladder and bowel programs and provide routine care for specific urinary and ostomy devices.
• Transfers patients and assists patients in mobility.
• Participates with skin management, prevention, positioning and treatment programs.
• Communicates all pertinent patient information and changes to licensed professional.
• Cleans and maintains equipment and environment and assures the area is stocked with appropriate supplies.

Provide 1:1 safety observation for cognitively impaired patients.
