The Assistant Team Leader may report to the Team Leader, APD, or PD of the designated program. The Assistant Team Leader shares a leadership role in the daily monitoring of implementation of assertive community treatment evidence‐based and recovery practices. The Assistant Team Leader is responsible for the direct supervision of assigned team members in order to ensure quality service delivery to members.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Supervise, guide and direct staff as assigned. Models evidence-based practices and recovery to ensure team adherence and understanding to fidelity scales.
  • Provide support for the management of all daily and/or weekly team meetings.
  • Meet with assigned team members to problem solve, train, monitor and guide their clinical case management and service delivery responsibilities toward effective, dignified and timely interventions.
  • Discuss with Team Leader, the activities of team staff members providing outreach, case management, social, vocational, education, physical well-being, and residential services and programming and other services and supports provided by the treatment team.
  • Provide direct service to members and their families.
  • Assist Team Leader with the monitoring of member attendance and/or occupancy on the team so that case assignments, transitions and closures are handled effectively and efficiently.
  • Ensure that the team functions optimally, in conjunction with other parts of the program while meeting the community support and rehabilitation needs of our members.
  • Ensure that all Thresholds procedures regarding the handling of member’s funds are followed and that both the members and the Thresholds administration are assured that all safeguards, reports and accounting meets the highest standards.
  • Participate in the implementation of Thresholds emergency plans related to extreme weather or other emergency condition.
  • Perform and participate in crisis intervention when necessary.
  • Monitor all required paperwork and maintain the quality and validity of individual case records according to CARF, Medicare, Medicaid standards and agency traditions. Ensure that all assigned paperwork is completed accurately and in a timely fashion.
  • Responsible for providing clinical direction as to the development and review of members’ documentation and critical forms, including the need to document the frequency of services.
  • Development and review requires face‐to‐face meetings with client or guardian to identify goals, objectives, special mental health service needs and the identification of staff responsible for delivering services.
  • Provide direct service to members through group counseling/therapy, case management, assertive community treatment, job support, crisis intervention, and psycho‐social rehabilitation services either in the community or on site at a Thresholds location.
  • May oversee or participate in the assessment of members to determine the need for service, identify and investigate available resources, explain options available to the client and assist in the application process.
  • Intervene with collateral sources, agencies, bureaucracies, facilities, and families on the members’ behalf. Advocate and assist with problem solving/resolution that will help the client access and utilize support from the community or family.
  • Participate in the implementation of a Thresholds Service Plan. This may require cooperation with the psychiatrist, other team members, the member, their families, and collaborating agencies.
  • Perform comprehensive member assessments in order to gage functioning levels and community supports.
  • Ensure that the Thresholds Service Plan addresses the member’s needs and includes objectives and techniques for accomplishing each goal.
  • Work cooperatively with inpatient hospital staff, keeping logs and other records of all contacts as required by agency practice.
  • Other duties as assigned.

MARGINAL DUTIES & RESPONSIBILITIES

  • May be required to work a flexible schedule.

EDUCATION

  • Bachelor's degree in social work, psychology, or closely related field required
  • Master's degree in social work, psychology, or closely related field preferred

EXPERIENCE

  • Three (3) years of prior clinical experience preferred.
  • Ability to synthesize and summarize information and make judgments regarding member care.
  • Computer proficiency, including the ability to use email, navigate and enter notes into electronic medical records in a timely manner.

SKILLS/CERTIFICATIONS

  • Current and valid driver’s license required.
  • Daily access to a personal vehicle is required. Must obtain and maintain $100,000/$300,000/$100,000 liability insurance coverage. Subject to validation every 6 months.
  • May be required to provide transportation for members of the program using a Thresholds provided vehicle. If required, must be 23 years of age and must meet all insurance carrier’s requirements. Subject to annual Department of Motor Vehicles license verification.
