Job Summary
The Certified Nursing Assistant (CNA)is responsible for the care of the residents in a designated area of the facility. The CNA is knowledgeable of the information as outlined on the Interdisciplinary Plan of Care and follows the plan. The CNA provides information to keep the plan of care current and communicates with the Licensed Staff and other departments regarding the resident's needs. The CNA provides care for each resident in the assigned group while maintaining the residents mental, physical and psychosocial needs. The CNA provides care to assist with keeping the resident well-groomed and clean. The CNA also assists to keep the resident care areas clean, in good working order, and promotes a home like atmosphere for the resident. The CNA reports all relevant observations to the Licensed Staff assigned to the unit in a timely manner and documents information timely as indicated. The CNA attends in-services and staff meetings as required. The Nursing Assistant is responsible to the Licensed Staff assigned to the specific unit.

Has a desire to work with the geriatric clientele as well as residents of any age and their current and prospective needs.
Must be in the facility ready to begin assigned duties on time and as scheduled.
Must be willing to work preferred shifts as scheduled and assist on other shifts as needed to assure adequate staffing to meet the needs of each resident.
Must have the ability to provide the care as outlined on the Interdisciplinary Plan of Care and other assignments for each resident in assigned group.
Must demonstrate ability to function as a cooperative team member with each member of the Nursing Department and other departments.
Must have ability to provide related resident care services as per facility policy and procedure.
Must display ability to adapt to on-the-job training and adhere to standards of the facility providing personal care services, assisting residents, caring for equipment and supplies and developing good understanding of basis of aseptic and sterile techniques.
14.Must be able and willing to function appropriately in a disaster or evacuation of the facility.
Must demonstrate ability to follow direction of supervising staff.
Makes rounds toward end of shift to assure area is in order for the oncoming shift.

Responsibilities
It is to be understood that the Certified Nursing Assistant (CNA) works by general, consistent, pre- arranged "Primary" group assignments as the basis of the activity for the shift, but must be able to accept changes and adapt to added assignment as the need arises. Also, each CNA must work with each other to assure safe practices for each resident and staff are followed. The Certified Nursing Assistant works with the Director of Nursing to assure the following responsibilities are accomplished. This list is only partial and should not be interpreted as a comprehensive inventory of all duties, tasks and responsibilities.
Signs in on Unit Assignment Sheet.
Reviews any changes on assignment sheets.
Receives report from Licensed Staff assigned to the unit
Checks residents and makes rounds on assigned group noting significant issues for
intervention. Begin planning time to assure all resident's needs are met throughout the shift.Assists residents as indicated to provide A.M., P.M. and daily care to assure residents are clean, dry, toileted, well-groomed and cared for to ensure quality of care.
Follows resident care needs as outlined on the Interdisciplinary Plan of Care.
Provides for privacy and dignity at all times.
Assists residents with meal activity including positioning, assistive devices, as well as hand, face, and dental hygiene before and after meals.
Assists in dining areas as assigned. Offers assistance to residents as needed. Offers substitutions for items not eaten well by any resident. Promotes atmosphere conducive to increased nutritional intake for each resident in the dining room.
For those resident's unable to go to the dining room, assists in meal service as assigned.
Assists residents to plan their time to enable them to be well cared for as well as attend their desired functions during the shift. (i.e. activities, visits with each other and their families, outings, meals, quiet times, etc.)
Answers call lights timely assisting with care as indicated.
Encourages and motivates residents to participate in recreational activities as well as activities of daily living.
Provides and keeps fresh water and fluids available for the residents at all times. Offers fluids to resident as assist as needed.
Timely serves and provides assistance with between meal nourishments as indicated.
Follows bath and whirlpool schedules as outlined for each resident.
Assist to provide personal hygiene (comb hair, brush teeth, etc.) during shift to promote resident appearance, cleanliness and quality of life.
Checks all residents frequently and at least every 2 hours assuring the resident is repositioned, turned, toileted, clean, safe, etc.
Provides intervention as needed to provide a safe clean environment.
Checks each resident who is being physically restrained releasing the restraint and assisting the resident to toilet at least every two hours
Checks all residents daily for condition of skin, reporting findings to Licensed Staff assigned to unit.
Assists licensed staff when needed to position residents in need of treatments or care.
Provides positioning devices as needed and as indicated for each resident.
Assists residents to bed for naps or night sleep as needed while providing a quiet restful and safe atmosphere to promote resident rest.
Takes and records vital signs as indicated.
Assists with weekly and monthly weights as assigned.
Follows infection control guidelines.
Assists the residents in keeping closets clean, clothing clean and in good repair, marked with their name, etc.
Assists residents to keep bathrooms clean, following infection control and privacy practices.
Changes residents' bed linen to assure the bedding is clean, not wrinkled, and promotes good hygiene and care practices.
Follows appropriate infection control practices in working with the residents and equipment.
Keeps accurate records of intake and outputs and procedures throughout the shift for those residents who require such.
Records information timely
Keeps Licensed Staff informed of pertinent information timely.
Assists other staff members as needed throughout the shift. Cooperates with other personnel to be sure residents are of prime importance to all members of the staff.
Assists new staff members in orientation as needed throughout the shift.
Maintains safe environment at all times, wiping up spills, working with housekeeping staff when available picking up articles as noted, etc.
Reports immediately to supervisor situations involving abuse, neglect, or suspicion of a crime.
