Suffield House Rehabilitation and Health Care Center is a 128-bed skilled nursing center that is seeking full and part-time CNA talent for the following schedules:
Full Time, 32 hours, Part Time, 16-24 hours and Per Diem on all shifts, with preference for 1st shift!
Shift differential of $0.50 per hour is added for 2nd & 3rd shift worked hours; $3.00 per hour is added for worked weekend hours.

POSITION REQUIREMENTS
Education / Experience
- Licensed Practical Nurse licensed in the State of Connecticut.
- Minimum of one year of education or experience in geriatric nursing or rehabilitation preferred.
- CPR certified.
- Ability to read, write and comprehend English; ability to follow oral and written instructions.
- Fluent knowledge of the MS Office suite and other office equipment.
- Ability to work hours as scheduled based on the requirements of the position assignment.
Working Conditions: Works in an appropriately lighted and ventilated environment.
Physical Requirements:
- Must be able to move intermittently throughout the day, proper body mechanics required. Ability to perform physically demanding worked involving frequent bending, stooping, turning, stretching, and reaching above the shoulders are involved.
- Must be able to see and hear or use prosthetics/equipment that will enable these senses to function adequately to assure that the requirements of this position can be fully met.
- Must be able to lift a minimum of 50 lb
ESSENTIAL FUNCTIONS
1. Administers medication and treatments according to the physician's orders.
2. Assumes responsibility for ordering medications.
3. Transcribes, documents, and implements physician's orders.
4. Collects data to assist in the nursing assessments during each shift.
5. Gives daily shift report to CNAs assigned to the unit prior to the start of the shift.
6. Reviews written daily assignments for each resident on assigned unit.
7. Assures assignments are completed before the end of the shift.
8. Gives and takes nursing report on assigned unit. Communicates essential information to appropriate parties.
9. Makes rounds on assigned residents at least once every two hours.
10. Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a regular basis and as the resident's condition changes.
11. Attends and participates in interdisciplinary resident care conferences as requested.
12. Assists the attending physician with rounds and special procedures as needed.
13. Participates in the yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps that need to be taken to improve their performance.
14. Assists Staff Development with orientation and on-the-job training for new nursing personnel as needed.
15. Collaborates with nursing supervisor in documenting disciplinary concerns for nursing assistants on designated shifts, including reprimands, oral and written warnings, and corrective action recommendations.
16. Notifies nursing supervisor, resident’s physician, and family of a change in the resident’s condition or an accident/incident involving the resident.
17. Assumes any duties that are assigned by the RN Supervisor/ADNS to provide optimal quality resident care.
18. Performs other related duties and attends meetings as may be deemed necessary by Supervisor / Administrator and or Director.
