$65,000 - We are seeking a dynamic and experienced professional to serve as the Team Lead for our Assertive Community Treatment (ACT) team. The ACT Team Lead provides clinical and administrative oversight to a multidisciplinary team delivering community-based mental health services to individuals with serious and persistent mental illness (SPMI). This role ensures high-quality, person-centered care while maintaining fidelity to the ACT model.
Key Responsibilities:
- Provide daily leadership, supervision, and support to ACT team members
- Ensure adherence to ACT model fidelity and best practices
- Coordinate and oversee service delivery, including crisis intervention and community-based care
- Conduct team meetings, case reviews, and treatment planning
- Monitor documentation for accuracy, timeliness, and compliance with regulatory standards
- Support staff development through coaching, training, and performance evaluations
- Collaborate with community partners, hospitals, and referral sources
- Participate in on-call rotation as needed
- Ensure compliance with state, federal, and organizational policies
Qualifications:
- Masters degree in Social Work, Counseling, Psychology, Psychiatric Nursing or related field
- Active, unrestricted licensure in North Carolina (e.g., LCSW, LCMHC, LMFT, or RN with psychiatric experience)
- Minimum of 23 years of experience working with individuals with SPMI
- At least 1 year of supervisory or leadership experience preferred
- Strong knowledge of the ACT model and community-based behavioral health services
- Valid drivers license and reliable transportation
- Experience working on an ACT Team (preferred)
- Certification or training in evidence-based practices (e.g., Motivational Interviewing, CBT, Trauma-Informed Care)(preferred)
- Experience with electronic health records and documentation systems (preferred)
