Position Summary: The Source LGBT Center’s CMA/Patient Coordinator II is responsible for coordinating a range of patient and client-centered services. They are responsible for advocating for clients and patients, completing client care plans, and conducting annual reassessments to ensure eligibility and service delivery. The CMA/Patient Coordinator II is a key external representative of The Source LGBT Center. This position reports to the Sexual Health Services (SHS) Program Manager and SHS Clinical Manager. This position does not qualify for remote work.
