JOB SUMMARY:

Are you passionate about supporting families during an important time in their lives- caring for a newborn or welcoming a new child? SPCC is seeking a compassionate, dedicated professional to join our Healthy Families Monroe team. In this dual-role position, you will provide strengths-based, home-visiting services to families, offering parenting support, case coordination, advocacy, and connections to essential resources. You’ll make a lasting impact by building relationships that foster safe, nurturing environments for infants and young children.

HOURS: This is a part-time position consisting of 20 hours per week. With a combination of remote, field & in-office hours that will be scheduled between 9AM – 6PM Monday -Friday; Occasional weekend availability required. Paid Training will require 40 hours of availability for a duration of three weeks, either separate or consecutive weeks (dates TBD upon hire)

RESPONSIBILITIES INCLUDE:

  • Provide home-based, family-centered support to parents, infants, and young children, promoting healthy development and attachment.
  • Manage a caseload of 8–12 families in alignment with Healthy Families New York (HFNY) service delivery standards.
  • Deliver case coordination, parenting education, emotional support, advocacy, and referrals to community resources.
  • Maintain accurate, timely, and compliant documentation within the HFNY data management system (MIS).
  • Participate in required supervision, trainings, and meetings to support professional development and program fidelity.
  • Build and maintain collaborative relationships with community providers to support family needs.
  • Conduct intake assessments and support program events that promote caregiver-child bonding and engagement.
  • Occasionally assist with assessments for newly referred families
Requirements:

COMPETENCIES/SKILLS:

  • Strong knowledge of infant and early childhood development, including parent-child attachment and relational dynamics.
  • Demonstrated ability to build trusting, respectful relationships with culturally diverse families and multidisciplinary professionals.
  • Effective communication and crisis management skills, with the ability to respond calmly and apply sound, trauma-informed judgment.
  • Ability to maintain accurate, timely documentation in electronic systems (e.g., MIS) and meet compliance requirements.
  • Capacity for reflective practice and a demonstrated commitment to diversity, equity, inclusion, and humanity-centered, trauma-informed care.

QUALIFICATIONS:

  • High school diploma required; some college coursework preferred
  • 3–5 years’ experience in human services, especially with children and families
  • Knowledge of infant development and parent-child attachment
  • Lived experience similar to the families we serve is a plus
