What are the responsibilities and job description for the Assistant Case Manager position at The Sentinel of Amsterdam?

Seeking a talented care driven individual for the role of full-time Assistant/Associate Case Manager for our assisted living community. Our focus is providing our residents with the best care possible while keeping them safely in their home within our community.

Main focus is to oversee the resident’s needs and coordinate with their physicians, nurses and other healthcare providers to ensure a smooth transition and a consistent future.

The ideal candidate MUST have:

  • A Bachelor's Degree in Human Services, Case Management, Social Work or the equivalent.

We would prefer a candidate with:

  • A minimum of 2 years experience in Assisted Living or Nursing Home environment

Salary - $24 - $28 / hour.
