Position Title: Caregiver Support Specialist

Reports To: Caregiver Support Program Director

Purpose of Position

The Caregiver Support Specialist provides care consultations, supportive counseling, and education to older adults and their caregivers, supplying appropriate resource information, coordinating referrals as necessary, and leading a weekly support group.

Responsibilities

  • Conducts care consultations and manages ongoing caseload of family caregivers
  • Responsible for follow-up activity.
  • Maintains appropriate documentation on client activity. o Provides coverage for necessary casework activity in the absence of other Caregiver Support Specialists.
  • Familiarizes self with resources, both locally and nationally. o Assists the director with preparation of all required reports and prepares monthly progress reports.
  • Assists director with the outcome evaluation of the program.
  • Assists in the planning and implementation of special seminars and forums.
  • Helps develop appropriate program content for targeted audiences.
  • Collaborate with CSP Director to create opportunities for presentations, forums, and community events.
  • Builds relationships with professional organizations to further expand program.
  • Outreach to various diverse communities to increase client base.
  • Attends regular agency staff meetings, component staff meetings and in-services trainings.
  • Represents the agency at meetings in the community, as needed.
  • Performs other duties that may be assigned.

  • Work compassionately and effectively with older adults, family caregivers, colleagues, and other professionals.
  • Great interpersonal and communication skills.
  • Culturally competent with strong understanding of the intersectionality of racism and poverty.
  • Excellent technology skills, with proficiency in Salesforce or other CRM platform required.
  • Ability to carry out assignments and projects without detailed instructions.
  • Ability to thrive in a fast-paced, deadline-driven environment.

Qualifications
Education/Experience :
A master’s degree in social work or related field of study from an accredited university, a licensed master level social worker preferred. A minimum of a Bachelor’s degree in a human services field and at least 2 years’ experience in the field of aging and/or social services with experience working in a direct service capacity may be substituted in lieu of a master’s degree.

Job Type: Full-time

Pay: $45,000.00 - $50,000.00 per year
