Key Responsibilities:
- Develop and implement individualized care plans in collaboration with interdisciplinary teams.
- Evaluate patient progress and adjust care plans to meet evolving needs.
- Serve as a liaison between patients, families, and the healthcare team to ensure continuity of care.
- Assess patients' post-discharge needs and coordinate with external providers and community resources.
- Facilitate smooth transitions to home, assisted living, or other care settings.
- Educate patients and families about available resources and services to support recovery and well-being.
- Maintain accurate and timely patient records, ensuring compliance with federal and state regulations.
- Prepare reports and participate in audits to monitor care quality and adherence to standards.
- Provide emotional support and counseling to patients and families.
- Advocate for patient needs and preferences in care planning and discharge processes.
Qualifications:
- Current Registered Nurse ( RN ) or Licensed Clinical Social Worker ( LCSW ) certification in the state of AL
- Minimum of 3 years in case management or discharge planning, preferably in a healthcare or long-term care setting.
- Strong organizational, problem-solving, and decision-making abilities.
- Exceptional communication and interpersonal skills.
- Proficiency in electronic medical records and care coordination systems.
