What are the responsibilities and job description for the Service Coordinator position at The Salvation Army Central Territory?

The Service Coordinator will link tenants of The Shield Apartments to supportive services provided by public agencies or private practitioners within the general community. Assess service needs to determine client eligibility and act as a liaison between The Salvation Army and the management company operating The Shield Apartments.
About the role:
- Acts as a liaison on behalf of tenants between The Salvation Army and the management agency operating The Shield Apartments.
- Develops the objective of The Shield Apartments “voucher program,” and creates consistency across client evaluation materials.
- Coordinates move in and move out for voucher program participants at The Shield Apartments.
- Creates community resource materials and events for program participants and community members. Maintains a directory of providers for use by tenants and project staff members.
- Tracks rental and utility income for the voucher program in collaboration with the local finance department.
- Recommends and facilitates administrative processes regarding The Shield Apartments to the local finance and social services management teams.
- Assesses voucher program participants’ income to establish consistent rental and lease agreements.
- Makes recommendations for maintenance needs to The Shield Apartments management company on behalf of tenants.
- Conducts casework interviews with clients seeking enrollment into the voucher program to obtain information for formulating program/service eligibility and case status.
- Provides general case management to facilitate positive long-term outcomes.
- Identifies psychosocial, economic, and physical needs of voucher participants; assesses and reassesses client's support systems, available community resources and other factors to plan, develop, and implement an appropriate service plan.
- Develops and implements a case plan that presents intervention strategies to meet voucher participant needs.
- Prepares complete and accurate case notes; writes correspondence, reports, and other written materials; inputs data into HMIS.
- Regularly peer review and audit of case files to ensure of services standards are met and contract compliance.
- Assists in transporting clients and advocating for them with public and private agencies.
- Attends meetings with other community service providers on behalf of The Salvation Army, as assigned.
- Conducts routine visits to tenants enrolled in the voucher program based on program expectations. Collaborates with other case managers and tenets residing at The Shield Apartments.
Education: Bachelor’s in social work, human services, psychology, sociology, or criminal justice.
Experience: Three years social work experience. Management experience helpful.
Certifications: Valid Wisconsin Driver’s license with approval to drive from The Salvation Army’s insurance.

Full-time employees scheduled to work 30 or more hours per week will be eligible for full benefits, including health, dental, vision and hearing benefits, basic and voluntary life benefits, holiday pay and paid time off, which is pro-rated based on their regularly scheduled work week.
Exempt Employees: Health, dental, vision, and hearing benefits, as well as basic and voluntary life benefits, will begin on the first day of the month following 30 days of employment.
