Overview

Duties
- Provide personal care services including assistance with daily living activities such as bathing, grooming, and dressing.
- Medication Reminders as prescribed and ensure adherence to care plans.
- Observe and report changes in patients' conditions or behaviors to healthcare professionals.
- Prepare nutritious meals tailored to individual dietary needs and preferences.
- Assist with mobility and lifting of patients as necessary, ensuring their safety at all times.
- Engage patients in meaningful activities that promote mental and emotional well-being.
- Maintain a clean and safe environment within the home or group home setting.
- Collaborate with family members and other healthcare providers to ensure comprehensive care.
Requirements
- Previous experience in senior care or home community care is preferred.
- Knowledge of dementia care practices is highly desirable.
- Familiarity with medication administration protocols and care plans.
- Ability to lift and assist patients safely while adhering to proper techniques.
- Strong observational skills to monitor patient health and report any concerns effectively.
- Excellent communication skills, both verbal and written, for effective interaction with patients, families, and team members.
