Registered Nurse

Job Overview
We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will have some experience with home healthcare. The Registered Nurse will play a vital role in ensuring patient safety, comfort, and compassionate communication.

Duties

• Perform comprehensive patient assessments including vital signs, home care IADLS/ADLs evaluations.
• Maintain patient files for various home care programs.
• Manage airway support including ventilator management and oxygen therapy.
• Coordinate care planning and work with case management to ensure seamless care and quality home care.
• Provide patient education on health coaching topics such as nutrition, medication management, chronic disease management (e.g., diabetes), home health saftey etc.
• Coordinate PCP paperwork, client charts are meeting regulations.

• Qualifications
• Valid RN license with current registration in the state of practice.
• Strong knowledge of medical terminology, anatomy & physiology, ICD coding (ICD-9 & ICD-10), CPT coding, and medical documentation standards.
• Ability to work effectively with diverse patient populations including dementia or Alzheimer’s disease/dementia care needs.
• Excellent communication skills combined with critical thinking abilities to perform diagnostic evaluations and develop individualized care plans.
• Must pass a comprehensive background check and drug screening prior to employment. Pay: $32.00 - $34.00 per hour

Expected hours: 40.0 per week
