Guided by our Self-Sufficiency Model (SSM) and a Housing First approach, we provide resources, advocacy, and compassionate support to help our guests achieve long-term stability.
Position Summary:We are seeking a dedicated and compassionate Case Manager to join our team. The Case Manager will provide direct support to shelter guests, develop individualized service plans, connect guests to community resources, and monitor progress toward self-sufficiency goals.
Key Responsibilities:
- Conduct intakes and assess the needs of shelter guests.
- Develop, implement, and monitor individualized case plans.
- Provide counseling, support, and advocacy for guests.
- Connect guests with housing, employment, healthcare, and social service resources.
- Maintain accurate and timely records of services provided.
- Collaborate with community partners and internal teams to ensure coordinated care.
- Monitor and evaluate program outcomes to improve service delivery.
Qualifications:
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
- Minimum of 2 years experience in case management or related social services.
- Strong knowledge of community resources and housing programs.
- Excellent interpersonal, communication, and organizational skills.
- Ability to work independently and as part of a collaborative team.
