Position Summary

Coordinates the Diabetes Education Program to serve population needs in accordance with goals outlined in the Diabetes Grant (SPDI). Provides leadership, assistance, support and coordination of services for community members of the Pueblo of Sandia in all areas of diabetes health education, wellness and prevention activities.

 Essential Duties and Responsibilities

  1. Plans, designs, implements and assesses a systematic comprehensive diabetes program in collaboration with Health/Wellness Center staff.  Serves as the Diabetes Team Leader.
  2. Writes, manages, and reports on State/Federal grants as appropriate.
  3. Develops and manages annual budget.
  4. Establishes strategic preventative health goals for all ages of community members.
  5. Creates marketing and outreach plans for the diabetes program.
  6. Develops Key Performance Indicators to track program goals.
  7. Provides individual and group education as necessary and or requested by team members on diabetes care (provider-oriented), self-care and management (client-oriented) and diabetes prevention (community-oriented).
  8. Works with the Clinic/Wellness staff on an on-going basis in developing and implementing diabetes prevention programs including health fairs, diabetes screening, education activities and exercise programs.
  9. Coordinates diabetes care and patient visits with Clinic/Wellness departments for the prevention of diabetes related complications; departments include Optometry, Dental, Physical Therapy, Nutrition, Podiatry, Senior and Recreation programs.
  10. In coordination with the Medical Director, assesses, plans, develops, implements and evaluates quality diabetes care for the community through annual in-depth diabetes program assessments.
  11. Case management and scheduling of appointments according to GPRA indicators for Diabetic patients.
  12. Works with IHS DM staff and becomes familiar with IHS audit tools and webpage and DM Registry.
  13. Performs additional duties and responsibilities as necessary or assigned.
  14. Punctual and regular attendance is an essential responsibility for all team members and are expected to report to work based upon the assigned schedule each day in accordance with the Attendance Policy for Pueblo of Sandia. 

 Additional Responsibilities

  1. May cross trained clinic nurse and RPMs’ package owner if appropriate.
  2. Demonstrates strong interpersonal skills in building a cohesive working relationship with co-workers and internal/external contact.
  3. Performs other job-related duties as assigned.

 

 Key Performance Indicators

This position has Key Performance Indicators (KPI’s) identified as a measurement of success.  KPI’s will be shared with all team members who work in this job title and feedback will be provided regarding the successful completion of the KPI’s as part of the performance review process.

Knowledge, Skills, and Abilities

  1. Knowledge of diabetes and healthcare prevention programs including physical activity, nutrition and diabetes management.
  2. Ability to work with a multi-disciplinary team of health care providers.
  3. Knowledge of community and community resources.
  4. Ability to give oral presentations and prepare written reports.
  5. Ability to collect, analyze and report data.
  6. Knowledge of Indian Health Service and other health services.
  7. Ability to travel and attend training seminars, continuing education programs, meetings and conferences.
  8. Basic computer knowledge using Windows based computer and Microsoft Office software.
  9. Experience working with children and seniors.
  10. Ability to create an effective marketing campaign.
  11. Ability to foster a positive relationship with community members.
  12. Ability to role model healthy behavior.
  13. Able to comprehend and abide by complex policies and procedures. Employees will adhere to the Pueblo of Sandia Human Resources Team Member Handbook and other PSHC Administrative policies, procedures and SOPs.
Qualifications:

   Education and Experience  

Required:

  1. High School Diploma, GED certification or equivalent.
  2. Must have Bachelor of Science in Nutrition.
  3. Five (5) years of relevant work experience in the health field with special emphasis required in community health education.
  4. Must have at least 2 years of experience in Tribe.

Preferred:

  1. Master’s degree in nutrition or related field.

Note:  Relevant work experience or education may be substituted to satisfy education and/or work experience. 

License/Certifications/Registrations

  1. Licensed nurse or Certified Diabetic Educator.
  2. Must possess and maintain a valid, unrestricted New Mexico Driver’s License.
    • Insurable driving record with no DWI/DUI violations in the last 5 years.
  3. Current CPR, First Aid and Blood Born Pathogen training certifications are required withing 30 days of hire date.
  4. Must successfully complete the New Mexico Food Handlers course within 30 day of hire date.
  5. The following immunizations and titers are required prior to employment start date:
  • Hepatitis B series or immune by titer
  • Tdap
  • MMR or immune by titer
  • Annual TB test
  • Annual Flu vaccination (seasonal)
  • COVID booster (optional but encouraged)
  1. Must be able to successfully pass a stringent background investigation and character investigation in compliance with PL 101.630.
  2. Will require a pre-employment and random drug screening.

Perceiving the nature of sounds at normal speaking levels with or without correction. Ability to receive detailed information through oral communication, and to make the discriminations in sound.

Bending legs at knee to come to a rest on knee or knees.

Lifting

Raising objects from a lower to a higher position or moving objects horizontally from position-to-position. This factor is important if it occurs to a considerable degree and requires substantial use of upper extremities and back muscles. (Up to 25 lbs.)

Pulling

Using upper extremities to exert force to draw, haul, or tug objects in a sustained motion. (Up to 25 lbs.)

Pushing

Using upper extremities to press against something with steady force to thrust forward, downward, or outward. (Up to 25 lbs.)

The ability to perceive the nature of objects by the eye.

Job requirements include, in the performance of duties, walking throughout the work area, on various work surfaces throughout internal or external locations.
