Position Overview
• Provide comprehensive outpatient primary care services across the lifespan
• Manage acute, chronic, and preventive health conditions
• Perform annual wellness visits and health screenings
• Coordinate care with specialists and hospital-based providers
• Utilize EMR for timely and accurate documentation
• Participate in shared call coverage (phone-only, minimal burden)
• Collaborate with advanced practice providers and support staff
• Opportunities to participate in academic and quality improvement initiatives
Qualifications
• Board Certified or Board Eligible in Family Medicine
• Eligible for Connecticut State medical licensure
• Active or eligible DEA registration
• Strong clinical and communication skills
• Commitment to patient-centered, team-based care
