Nurse Practitioner – Palliative Care
Location: Jacksonville, NC
About Us:
Seeking an experienced Nurse Practitioner to deliver compassionate care while building community partnerships.
Schedule:
- Monday–Friday, 8 AM – 5 PM
- No weekends, no on-call
Key Responsibilities:
- Provide palliative care in homes, assisted living, and skilled nursing facilities (4–5 visits/day).
- Lead a weekly palliative care clinic.
- Educate patients/families on disease management, medications, and advance care planning.
- Coordinate care with physicians and the interdisciplinary team.
- Participate in community outreach, education, and referral development.
Qualifications:
- Master’s Degree in Nursing, NP certification (AANP/ANCC).
- NC Nurse Practitioner license (or eligibility).
- 5 years medical experience, 4 years NP experience.
- Palliative care experience required.
- Valid driver’s license/reliable vehicle.
- Preferred: Hospice/palliative certification, home care or hospital experience, program development skills.
Compensation & Benefits:
- Salary: $105K–$148K (based on experience).
