General Duties
- Independently manage an active caseload of approximately 25–30 members receiving assistance with housing support services.
- Work with clients to develop individualized Housing Stabilization Plans, and document Levels of Need Assessments.
- Assist clients with identifying goals related to housing such as document acquisition. increasing income, employment, benefits, budgeting, and long-term self-sufficiency.
- Monitor client progress through traveling to scheduled in-person visits, in person office visits, remote phone and zoom calls, and post-exit follow-up.
- Maintain and document accurate, timely, and Medicaid-compliant case notes, service plans, referrals, and progress documentation.
- Provide resources through understanding the availability and details of community providers, behavioral health professionals, social service agencies, and housing programs.
- Participate in regular staff meetings and required training.
- Perform other related duties as assigned.
Pre -Tenancy Services
- Maintain and update individualized housing support plans, with measurable short and long-term goals.
- Maintain and update housing support plans and level of needs assessments as necessary.
- Assist members with completion of identified goals such as resume assistance, job applications and suggestions, creation and completion of budgeting documents, benefit applications, housing/apartment searches and applications, voucher applications, etc.
- Assist members with landlord and property management communications including inspection processes, and lease agreements (understanding and singing).
- Maintain the above and support the client by maintaining regular in person and over the phone visits.
- Properly document all visits and communications with the member through the proper channels for both Medicaid and the agency
- Be proficient with the use of applications such as slack, outlook, and adobe.
Tenancy-Sustaining Services
- Maintain and update individualized housing support plans with measurable short- and long-term goals.
- Maintain and update level of needs assessments as needed.
- Support independent living skill development including house keeping, budgeting,time management, and community resources/integration.
- Assisting the member in gaining access to resources for utility assistance, income support, and other public benefits needed to maintain housing stability.
- Maintain the above and support the client by maintaining regular in person and over the phone visits.
- Ultimately providing ongoing support to help members maintain successful long-term tenancy and community integration.
Required Qualifications
- Minimum of 2 years of direct case management experience in housing services, supportive housing, homelessness services, rapid rehousing, shelter services, housing navigation, or other tenancy support programs is required.
- Bachelor’s in social work or related field required.
- Experience managing client caseloads and developing individualized service or housing stabilization plans.
- Strong knowledge of New Jersey housing resources, landlord engagement, community referrals, and housing assistance programs.
- Experience working with Medicaid populations, individuals experiencing homelessness, behavioral health populations, or individuals with complex social needs is strongly preferred.
- Ability to complete all necessary tasks and documentation independently and accurately to maintain compliance with program and Medicaid requirements is preferred.
- Strong communication skills, organizational skills, crisis management, and client advocacy required.
