Social Services Director

The Pines Rehabilitation & Health Center is a 60 Bed SNF/NF facility that provides Quality and Compassionate Resident Centered Care to the residents we serve. We're located in Beautiful Lyndonville, VT. We are currently accepting applications for a Full Time Social Services Director.

As the Social Services Director, your primary role includes:

• Collaborating with the interdisciplinary team, nursing and administration to promote and protect patient rights, dignity, independence and psychological well-being
• Maintaining good working relationships with center employees, residents and families
• Assuring a comprehensive Psychosocial Assessment is completed for each patient that identifies social, emotional, psychological needs and strengths.
• Educating patients and families regarding their rights and responsibilities, health care decision making/advance directives, effective problem solving, and available community resources
• Providing therapeutic interventions to assist patients to cope with their transition and adjustment to a long-term care facility and to address behavior or mood problems
• Working with facility personnel, families and residents in completing any necessary insurance enrollments, prior authorizations and coordination of Medicaid applications.

Candidates must have the following qualifications:

• Bachelors Degree in the field of Social Services/psychology preferred
• A desire to be an active voice and advocate for the residents and neighbors of the community we serve
• Basic computer and software skills required
