Beautiful Assisted Living facility with stable staffing in Troy, NY is seeking a Director of Nursing.
This is a great opportunity for an RN with some management experience to step in and take the reins in a beautiful Majority-Medicaid Assisted Living Facility.
The Pines at Heartwood, a member of the National Health Care Associates Family of Facilities with 44 centers in 6 states, runs a Medicaid approved program wherein we provide all the comforts of home with personalized care that supports health and independence.

The facility is conveniently located near Samaritan and St Mary’s Hospitals with easy access from all major thoroughfares and bus lines.
What You'll Do:
Key Responsibilities:
- Works with the Executive Director to develop, allocate, and control the budget for the Nursing Department.
- Activily participates in the center’s strategic marketing plans.
- Recruit, hire, train, supervise, and evaluate nursing and caregiving staff.
- Provide clinical education, mentoring, and performance feedback to ensure high standards of care.
- Maintains a system to ensure timely completion of medical evaluations and UAS.
- Develop, implement, and update individualized service plans based on resident needs and preferences.
- Ensure compliance with state and licensing requirements governing the assisted living facility.
- Conduct audits, quality assurance reviews, and corrective action plans as needed.
- Prepare for and participate in licensing inspections and surveys.
- Communicate effectively with residents and families regarding care needs, changes in condition, and wellness programs.

- Graduate of an accredited school of nursing, college, or university with a current registered nurse licensure by the State Board of Nursing.
- Minimum 2-3 years of nursing experience, preferably in assisted living, long-term care, or home health with prior management experience.
- Certified in the Uniform Assessment System (UAS) or eligible within 30 days of hire.
- Strong leadership, communication, and organizational skills.
- Knowledge of state and licensing regulations.
