POSITION PURPOSE:
The Individualized Home Support Professional provides essential support and skill-building for individuals living in their own or family homes, enhancing their ability to live independently in the community.
NATURE AND SCOPE:
The IHS Professional reports to the Program Director, who reports to the Vice President of Community Services, who reports to the President. The IHS Professional is responsible for teaching clients independent living skills that allow and enhance their ability to live independently in the community. It is necessary that the IHS Professional have knowledge of a variety of disabilities and chronic illnesses and experience in person-centered approaches. Services include support and training in community living service categories, which may include community participation, health, safety and wellness, household management, and adaptive skills. The work environment requires travel among other agencies, and flexible work hours. This is a non-exempt position.
JOB RESPONSIBILITIES:
- Individualized Home Support Services
- Provides support and training in at least one community living service area, including community participation, health, safety and wellness, household management, and adaptive skills.
- Assessment and Program Planning
- Conducts assessments and coordinates Individualized Home Support program plans.
- Provides assistance, supervision, and training in independent living.
- Coordinates services between client and outside agencies (vocational, rehabilitative, cultural, recreational, and medical services).
- Acts as an advocate for individuals and their needs.

- Provides support and training in coordination of transportation as necessary.
- Responsible for completing and maintaining documentation for Individualized Home Supports with Training services.
- Customer/Human Relations
- Communicates effectively with clients, families, outside agencies, and coworkers.
- Maintains cordial relationships and optimal communication.
- Displays positive, supportive, and cooperative conduct.
- Communicates effectively verbally and in writing.
- Listens accurately and responsively.
- Seeks/pursues services for non-English speaking clients as appropriate.
- Documentation
- Documents information pertinent to client services in compliance with guidelines and regulations.
- Completes and maintains all documentation required for optimal communication with clients and family members.
- Completes all documentation necessary for billing Individualized Home Supports with Training services in the medical assistance program.
- Develops reports as required for the county.
- Regulatory Compliance
- Adheres to the policies, procedures, and philosophy of The Phoenix Residence, Inc.
- Staff Development
- Attends all mandatory and job-related in-services.
- Maintains working knowledge of a variety of disabilities and special health needs, as well as community supports.
- Maintains knowledge of symptoms and effects of disabilities and chronic illnesses.
- Receives ongoing training regarding supervision, training, and assistance with independent living skills services.
QUALIFICATIONS
- Bachelor of Arts preferred.
- At least 1 year of experience with persons with disabilities and/or in Health and Human Services.
- Experience with person centered planning approaches.
- Knowledge of disabilities/chronic illnesses.
- Knowledge of community living skills management.
- Excellent communication skills, written and verbal.

- Ability to travel to client homes and among agencies; if driving, holds a valid license and maintains a safe driving record.
- Ability to work with diverse clients.
- Ability to work flexible hours.
Salary : $17 - $20
