The Phoenix Rehabilitation and Nursing Center is looking for a good Social Worker to join our team.
Social Worker Job Description:
Looking for a full-time Social Worker that is self motivated with the ability to evaluate and treat independently without direct supervision. Must possess the desire to assist residents improve and should have a passion for working with geriatric population. Must be flexible, detail-oriented and have the ability to multi-task.
Social Worker Qualifications:
- Must have a bachelors in Social Work or related field- LTC MDS experience preferred.
- Must have the ability to work independently and multitask.
- Must have a positive attitude and be able to provide excellent customer service.
- Must be familiar with the Electronic Medical Records programs ; preferably PCC
Social Worker Functions:
- Meet with patient/residents and their family’s to gather information and design a safe, secure discharge plan for each resident..
- Complete all necessary forms/documents for each patient in a timely fashion as per facility dictates.
- Work with other departments to assist patients/residents in any way possible.
- Make community referrals for each discharged patient, as indicated.

Salary: Up to $70,000 a year based on experience
